SHOW Middleton & Milwaukee Survey Centers

SHOW has two fixed Survey Centers, one in Middleton and one in Milwaukee. SHOW also travels with Mobile Survey Centers for participants who do not live near the fixed centers.

Middleton

SHOW’s Middleton Survey Center is located at 6602 University Avenue in Middleton, Wisconsin. The city of Middleton is just northwest of the City of Madison.

Parking is free at the Survey Center.

Directions

Traveling East on Highway 14:
• Take Highway 14 East towards Madison/Middleton.
• As you head east into Middleton, Highway 14 turns into University Avenue.
• You will pass under the Highway 12 intersection.
• The exam center is roughly one mile after this intersection, just after the intersection with Mendota Avenue.
• SHOW exam center is on the left side of the street.
There is free parking available at the center.

Traveling North on Highway 12 or 14: 
• Head north towards Madison/Middleton on the Beltline Highway by US-12/US-14.
• Take exit 251A-B for US-14 W toward Spring Green/La Crosse.
• Turn left/east at University Avenue.
• The exam center is roughly one mile after this intersection, just after the intersection with Mendota Avenue.
• SHOW exam center is on the left side of the street.

Traveling South on 12:
• Head south towards Madison/Middleton on the Beltline Highway. (US-12/US-14).
• Take exit 251A for US-14 W toward Spring Green/La Crosse.
• Turn left/east at University Avenue.
• The exam center is roughly one mile after this intersection, just after the intersection with Mendota Avenue.
• SHOW exam center is on the left side of the street.

Milwaukee

The Milwaukee SHOW exam center is located at the downtown Milwaukee at 1218 W Kilbourn Ave Suite 101, between N 12th and N 13th streets.

Parking: Free parking is available south of the center on the southwest corner of Kilbourn and 12th St. The entrance to the structure is on 12th St. The SHOW Exam Center is in the Physician Office Building across the street from the parking structure. The Physician Office building can be accessed from the parking structure at street level or through a covered walkway on the 3rd floor.

• Take I-43 South into Milwaukee
• Take exit 72E toward Highland Ave
• Turn right at W Highland Ave/US-18
• Turn left at N 12th St
• Turn right at W Kilbourn St

Traveling from the South:
• Take I-43/I-94/US-41 North into Milwaukee
• Take exit 72C to merge onto W Kilbourn Ave
• Turn left at N 6th St/WI-145 W
• Turn left at W State St/US-18 W, Continue to follow W State St
• Turn left at N 12th St
• Turn right at W Kilbourn St

Traveling from Downtown Milwaukee:
• Take 18/State Street West, across the river
• Turn left at N. 12th Street
• Turn right at Kilbourn Street

Traveling from the West:
• Take I-94 East into Milwaukee
• Take exit 310A for 13 th St
• Turn left at N 13th St
• Turn right at W Tory Hill St
• Turn left at N 10th St
